Covid-19 Safety Measures

In order to minimize the amount of time you spend in the office, we will call you 1-2 days before your appointment to update medications and health history, the current eye problems and questions you may have for your doctor.

PLEASE LET US KNOW IMMEDIATELY IF YOU HAVE BEEN IN CLOSE CONTACT TO ANYONE WITH KNOWN COVID-19 OR IF YOU OR ANYONE LIVING WITH YOU EXHIBIT SYMPTOMS SUCH AS FEVER, COUGH, SNEEZING/RUNNY NOSE OR MUSCLE/JOINT PAIN

  • If you are a new patient to the practice we encourage you to download the registration forms and complete them prior to your visit. If you do not have access to the forms we will print them for you upon your arrival and ask you to complete the forms while you are waiting in the car.
  • You must wear a mask (surgical or cloth) upon entry into our office and throughout your stay.
  • In order to minimize exposure we have limited the number of patient appointments so fewer patients are in our office at any one time.
  • Plexiglas protection was installed in our office reception area.
  • Hand sanitizer is available throughout our office. You also may use the bathroom in the office to wash your hands before you enter the exam room.
  • Our exam rooms and testing equipments are sanitized based on the CDC’s latest recommendations. For your safety our exam and testing rooms are equipped with UVC-air purifiers.
  • Dr. Farrokh-Siar and her staff wear masks in the office.
  • When you arrive for your appointment, please remain in your car and call and leave a message at 708-429-3937 to announce your arrival. You will be checked in remotely.
  • If you arrive earlier than your appointment, please remain in your car. We bring patients in according to their appointment time.
  • We only permit patients inside our office and exam rooms. Please have your driver wait in the the car in the parking lot.
  • We can make an exception if you require assistance walking.
  • In order to minimize the number of occupants and exposure in the office we will ask you to enter the exam room immediately after your arrival and keep the waiting room vacant.
  • You will check out and are given your next appointment in the exam room.
