NEW PATIENT
If you are a new patient, please arrive 15 minutes early so that you can complete new patient registration forms and health history forms. If you were referred to us for a consultation, we recommend to have your previous records sent to us prior to your appointment. Please be sure to bring your list of medications and eye drops. Your primary visit will take approximately 1 1/2 hours. If you have any other questions or concerns, please give us a call. We look forward to meeting you!
HOW TO PREPARE
If you have already scheduled an appointment with us, please come prepared.
You will need to bring a valid ID, insurance card, referral form and previous records. If you are having your pupils dilated, be aware that your near and distance vision will be affected for at least three to five hours after dilation.
